Anaphylaxis after induction to general anaesthesia: Four case reportsAnaesthesiology - Case Reports
Frančáková Jana, Fiala Hynek
Anest. intenziv. Med. 2009;20(2):73-77
Allergic reactions during regional and general anaesthesia represent relatively infrequent but serious complications. Out of the drugs used in anaesthetics, the most common triggers of allergic reactions are muscle relaxants, particularly suxamethonium and rocuronium. Atracurium on the other hand usually causes anaphylactoid reactions. The authors present four case reports illustrating the anaphylactic and anaphylactiod reactions. The pathophysiology, diagnostics and management of serious allergic reactions are discussed. The authors also discuss the preoperative assesment and preparation of allergic patients and the preparation of the anaesthesiologists...
Anterior spinal artery syndrome as a complication of neuroaxial blocade - successful treatment with prostacyclin (case report)Anaesthesiology - Case Reports
Stibor Bronislav, Schwameis Franz, Fellner Hermann
Anest. intenziv. Med. 2009;20(2):78-83
The authors report anterior spinal artery syndrome occurring as a complication of neuraxial blocade. They describe the cause and origin of the syndrome, the clinical symptoms, illness trajectory and its successful treatment. The patient, a 66-year-old man, underwent colon resection of a colonoscopically detected carcinoma. The surgery was carried out under general anaesthesia. An epidural catheter at the L2/3 level was placed preoperatively for peri- and postoperative analgesia. The patient was extubated about 8 hours after catheter placement. Later on, total spinal paraplegia was detected corresponding to the level of the catheter placement. The 3D-CT...
Erythropoietin and other erythropoiesis-stimulating drugs and bloodless medicine - a new era in the management of anaemia?Anaesthesiology - Review articles
Stibor Bronislav, Schwameis Franz
Anest. intenziv. Med. 2009;20(2):84-87
Bloodless medicine tries to find and put into practice alternatives to allogeneic transfusions. The biggest risk when receiving a blood transfusion is the transmission of infectious diseases. Since the 1980s the recombinant human erythropoietin has been used in anaemia treatment, the first of the group of erythropoiesis stimulating agents. Its main disadvantage is the need for frequent applications; a very serious complication - pure red-cell aplasia - occurs rarely.A second generation drug - darbepoetin - features a modified chemical structure and more favourable pharmacokinetics. A third generation representative of continuous erythropoietin...

Objective:Assessment of the functioning of extra-pulmonary organs during the ventilation of healthy lungs. Type of study:Experimental, comparative. Setting:Experimental Centre of the University Hospital. Materials and methods:15 white piglets weighing 22.7 (18.8-27) kg were identically instrumentated and monitored under general anaesthesia and divided into three equal groups. The pigs in Group A breathed spontaneously, group B pigs were ventilated with VT 6 ml . kg-1 and group C pigs were ventilated with VT 10 ml . kg-1. Measurements and samplings were...
Hypocholesterolaemia on the medical intensive care unit: The process of cholesterol synthesis and absorption is altered in the acute phase of diseaseIntensive Care Medicine - Original Paper
Hrabovský Vladimír, Zadák Zdeněk, Bláha Vladimír, Hyšpler Radomír, Mendlová Alice
Anest. intenziv. Med. 2009;20(2):96-101
Objective:Hypocholesterolaemia has been described as a common feature in patients with acute disease and is associated with poor prognosis. By using cholesterol synthesis (lathosterol, squalene) and absorption (sitosterol, campesterol) markers it is possible to determine the changes in both the processes of cholesterol acquisition. The aim of this study is to describe the changes of cholesterol metabolism in patients with acute medical disease. Design:Prospective study. Setting:Department of Medicine, Teaching Hospital. Materials and methods:The study included 60 patients with acute...

Objective:Evaluation of the impact of the admission clinical status of subarachnoid haemorrhage (SAH) patients (Hunt Hesse Score, HH3), the outcome (Glasgow Outcome Score, GOS) and the presence of vasospasm on the changes in the brain tissue oxygen levels (PbtO2), in relation to changes of FiO2 in mechanically ventilated patients. Design:Prospective interventional study. Setting:Neurosurgical Department of University Hospital, Department of Anaesthesia and Resuscitation, University Hospital. Materials and methods:We monitored PbtO2 in patients with SAH due to aneurysm...
A fatal complication of tracheostomy in a 13-year-old patient with Duchenne Muscular Dystrophy - case reportIntesive Care Medicine - Case Report
Stříteská Jana, Pařízková Renata, Dostál Pavel, Hoffmann Petr, Černý Vladimír
Anest. intenziv. Med. 2009;20(2):107-112
The authors describe a case of a 13-year-old boy suffering from Duchenne muscular dystrophy. Permanent mechanical ventilation was necessary due to the patient's progressive chronic respiratory insufficiency. A surgical tracheostomy was complicated by the development of tracheal stenosis managed with special types of tracheostomy tubes. A tracheo-arterial fistula developed as a result of progression of skeletal chest deformities and the tracheal stenosis. The patient died of massive arterial haemorrhage from the fistula formed between the trachea and the brachiocephalic trunk.
