Anest. intenziv. Med. 2007;18(3):141-149
Objective:To test the hypothesis that patients with acute normovolaemic haemodilution (ANH) tolerate better aortic clamping and declamping during abdominal aortic aneurysm (AAA) repair. Perioperative haemodynamic changes are mitigated, thus the frequency of ischaemic events and cardiovascular complications are decreased in comparison with the group of patients without ANH. Design:Prospective randomized study. Setting:Department of Anesthesiology, Resuscitation and Intensive Medicine, University Hospital. Materials and methods:50 patients admitted for elective AAA repair were prospectively randomized...
Early resuscitation of septic shock to different levels of arterial blood pressureIntensive Care Medicine - Original Paper

Anest. intenziv. Med. 2007;18(3):150-156
Objective:The optimal target mean arterial pressure (MAP) in the resuscitation of septic shock is undetermined. The aim of this study was to assess the effect of different levels of MAP on splanchnic organ perfusion/ function and lactate metabolism. Design:Prospective, randomized, clinical trial. Setting:St. Anna University Hospital, Brno, Department of Anaesthesia and Intensive Care. Materials and methods:Patients in the first 6 hours of septic shock were randomized to target MAP of 65 (G65) or 85 (G85) mm Hg. The target MAP was achieved by the admisnistration of fluids and norepinephrine. A...

Objective:To investigate if the addition of synbiotics to enteral nutrition improves the prognosis, morbidity, immune status, enteral nutrition tolerance and gut integrity in the critically ill patient. Design:Prospective, open, placebo controlled clinical study. Setting:Department Anaesthesia and Intensive Care, St. Anna University Hospital, Brno, Czech Republic. Material and methods:Patients estimated to stay in the ICU > 7 days were randomised to either post-pyloric Synbiotic Forte (Medipharm, Sweden) or tea. The ICU mortality, ICU length of stay, nosocomial infections, occurrence of sepsis,...

Anest. intenziv. Med. 2007;18(3):164-169
Objective:To establish the predictive value of an extended spectrum of immunologic parameters measured during the initial ICU days on the ICU survival of patients requiring long-term intensive care. Design:Prospective, clinical study. Setting:Department Anaesthesia and Intensive Care, St. Anna University Hospital, Brno, Czech Republic. Material and methods:The immune profile was taken in patients estimated to stay in the ICU > 3 days in 5- day intervals. In this paper we report on the trends of TNF-alpha production of the whole blood after LPS stimulation (TNF-alpha), plasma IL-6 and IL-10...

Anest. intenziv. Med. 2007;18(3):170-174
Objective:To establish the predictive value of monocyte activation during the initial ICU days on the ICU survival of patients requiring long-term intensive care. Design:Prospective, clinical study. Setting:Department Anaesthesia and Intensive Care, St. Anna University Hospital, Brno, Czech Republic. Material and methods:The immune profile was taken in patients estimated to stay in the ICU > 3 days in 5-day intervals. In this paper we report on activation of monocytes (Dr locus expression i.e. CD14+HLA-DR+) determined by flow-cytometry during the first 5 days of ICU stay.We study...

The evolution of airway managementHistory
J. Pokorný
Anest. intenziv. Med. 2007;18(3):181-186
This article describes the most important steps of the evolution of the contemporary methods of airway management in anaesthesiology and critical care. In the 19th century the principal method of safeguarding the open airway was tracheostomy, cannulation of the trachea and supralaryngeal tamponade prior to the beginning of general anaesthesia and surgery. Trendelenburg (1869) was the first one to use an inflatable cuff for sealing the tracheostomy cannula. MacEwen introduced inserting the tracheal tube into the larynx through the mouth in 1878. The method was improved by Maydl in Prague (1893). Magill and Rowbotham developed a well usable...
