Anaesthesia information management systems in the Czech Republic - Are they still just a calling of the future?Celková anestezie - přehledový článek

J. Bruthans

Anest. intenziv. Med. 2019;30(1):3-8 | DOI: 10.36290/aim.2019.001

Anaesthesia information management system (AIMS) offers a wide range of benefits - automated record keeping, improved pre- and postoperative care and usage of clinical decision systems. Concerns about legal issues when using AIMS have not been proven. Although at least ten per cent of tertiary anaesthesia departments use some kind of AIMS in the developed countries, only three pilot implementation projects from different suppliers are in progress in the Czech Republic.

Anest. intenziv. Med. 2019;30(1):14-21 | DOI: 10.36290/aim.2019.003

Objective: The endothelial glycocalyx (EG) plays a central role in the coupling functions of microcirculation and tissue metabolism. EG is particularly important in critically ill patients in whom it suffers damage. The aim of study was to describe the condition of EG in critically ill patients in dependence of the severity of the illness and the usage of various methods of organ support, using a non-invasive method assessment. Design: Monocentric, prospective, observational study. Setting: Intensive Care Unit (ICU) at a University Hospital. Materials and methods: Adult patients admitted to the ICU with...

Pre-hospital diagnosis of interstitial lung disease in patients presenting with shortness of breath - case seriesCase Report

R. Škulec

Anest. intenziv. Med. 2019;30(1):22-26 | DOI: 10.36290/aim.2019.004

The author presents a case series of four patients presenting with shortness of breath who have been diagnosed with interstitial lung disease in the pre-hospital setting based on a combination of clinical examination and Point-of-Care chest ultrasound. Further specialized examinations in the hospital confirmed the diagnosis in all of them. The common primary diagnostic finding was a combination of severe shortness of breath, ultrasound finding of bilateral alveolo-interstitial syndrome with irregular distribution, bilateral weakening of pleural sliding and absence of echocardiographic signs of heart failure. The author discusses the potential contribution...

Clinical presentation and management of phantom painLéčba bolesti - přehledový článek

J. Lejčko

Anest. intenziv. Med. 2019;30(1):27-32 | DOI: 10.36290/aim.2019.006

Phantom limb pain is a relatively common and disabling condition with a negative impact on the quality of life. Simultaneously with this exceptional type of pain, stump pain frequently occurs. Phantom pain is considered neuropathic pain. Clinical picture of phantom limb pain is influenced by a number of factors - pre-amputation pain, site of amputation, gender, etc. The exact pathophysiological mechanisms of phantom pain have not yet been defined. Presumably, peripheral and central neuroplastic changes involving cortical reorganization are key factor in the genesis of phantom pain. The role of 'mirror' neurons in the prefrontal cortex has been proposed....

Caudal block in adult patients and its use in the perioperative periodLéčba bolesti - přehledový článek

K. Axmann, M. Pieran, E. Berta, T. Gabrhelík

Anest. intenziv. Med. 2019;30(1):33-38 | DOI: 10.36290/aim.2019.007

Caudal block is a well-known and thoroughly studied technique of neuraxial anaesthesia routinely performed in paediatric anaesthesia. In adults, however, this method has been used mostly in chronic pain management. Due to more detailed understanding of anatomy and the routine use of ultrasound guidance, caudal block has recently been reintroduced into perioperative care in adults. It is now considered a simple, safe and reliable technique for both anaesthesia and postoperative analgesia in this group of patients. This review describes the anatomy and technical aspects with a focus on ultrasound guidance as well as suitable applications of this block...
