We describe a case of a 49 years old female patient with acute liver failure caused by halothan induced hepatitis. The liver failure was so severe that she was indicated for liver transplantation. The successful outcome of liver transplantation was facilitated also by supportive treatment with fractionated plasma separation and adsorption (FPSA) on Prometheus® device.

A case report of acute theophylline overdose in a mentally retarded patient is presented. Due to alterations of consciousness, no relevant data enabling a potential diagnosis of theophylline intoxication could be obtained either from the patient or from his family. Considering methanol and ethylenglycol intoxication, toxicologic analysis was realized, that finally revealed a severe theophylline intoxication, consistent with both clinical and biochemical status. Acute disseminated encephalomyelitis (ADEM) is an immune-mediated demyelinisation of the brain and spinal cord, precipitated by a infection or vaccination. In the past ADEM was a rare disease with higher incidence recently. Although history, clinical and radiological presentations are typical for this disease, the diagnosis is made per exclusionem. ADEM requiring ICU admission is a severe disease causing high mortality and persistent disability. Authors describe course of illness in one patient, diagnostic process a and therapy with corticosteroids, IVIG a plasmapheresis.

Central venous catheters (CVC) take indispensable place in caring for critically ill patients. Catheter-related infections increase morbidity and mortality. In the article basic CVC protection methods are mentioned with accent on overview of catheters available on the market. Overviews of meta-analyses and several methodological flaws of recent studies are also referred. Meta-analyses showed that anti-infective CVCs reduce the risk of catheter-related blood stream infections, but their routinely use is still not recommended. However, the use of anti-infective CVCs should be considered, when background rates of catheter-related infections are high.

Acute heart failure syndromes comprises a heterogenous set of conditions associated with a high mortality and hospitalizations rates and a consequent heavy healthcare burden. Initial therapy in patients with acute heart failure should be directed at improving symptoms and patient haemodynamics. The evidence base for many traditional therapies is weak, and, in some cases, short-term benefits are gained at the expense of long-term outcomes. This paper summarizes current basic pharmacological and non-pharmacological management of patients with acute heart failure syndromes.

The author presents his reminiscence of the beginning of anaesthesiology in the ENT Department of the Faculty of Medicine, Charles University in Pilsen, as well as his own first experience in this field. He tries to record the scanty conditions of, at that time newly established branch, and the difficulties of the advancement of the ENT anaesthesiology in Pilsen 50 years ago. He describes the development of that ENT branch and mentions also briefly the outlines of knowledge of the resorptive potential of mucous membranes of the airways and its utilization in anaesthesiology.
