The effect of two different anesthesia regimens on annexin V levels and early postoperative complications in CABG
Maruniak S. R., Loskutov O. A., Druzhyna О. М., Malysh І. R., Korotchuk N. V.
Anest. intenziv. Med. 2022;33(3-4):135-140 | DOI: 10.36290/aim.2022.021
Introduction. Annexins are a family of phospholipid‑binding proteins that reversibly, specifically, and with high affinity bind to cells expressing phosphatidylserine during apoptosis. This phenomenon may be important in the pathogenesis of early postoperative cardiac complications. The aim of the study was to analyse the effect of anaesthetic strategy on the dynamics of annexin V during coronary artery bypass graft (CABG) with cardiopulmonary bypass (CPB) as well as the dependence of direct clinical results on the expression of annexin V. Materials and methods. The study included 30 patients with coronary artery disease (CAD) who underwent...

Anest. intenziv. Med. 2022;33(3-4):141-147 | DOI: 10.36290/aim.2022.028
Objectives: To evaluate the effectiveness of ARS - a newly developed instrument designed for advance care planning for patients with advanced serious illness - in clinical settings, to examine potential differences in perceived effectiveness of ARS between experts and practitioners, and to determine areas in which ARS is perceived as most useful. Design: Survey; descriptive and correlational study. Setting: Departments of anaesthesiology, resuscitation, intensive medicine, neurology, infectious diseases, and internal medicine, at university and district hospitals. Material and methods: Ten experts with extensive experience in the field and 20 practitioners...
Aspiration pneumonia after cerebrovascular stroke: a comparison between patients with and without dysphagia
Moulaei N. A., Ramroodi N., Tabatabaie S. M. N. A., Danesh H. A., Khazaie H. A.
Anest. intenziv. Med. 2022;33(3-4):148-152 | DOI: 10.36290/aim.2022.023
Introduction: Pneumonia develops in 11%-21% of patients with a history of acute stroke and weakens their system conditions. The main cause of pneumonia is aspiration, and there is a relationship between dysphagia, which occurs in pathological conditions, and disorders of consciousness. This research aims to evaluate and compare the prevalence rate of aspiration pneumonia in stroke patients with and without dysphagia. Methods: We retrospectively identified patients who had experienced a stroke from March 1, 2018, to December 31, 2021 through their documented data. Then, we compared the dysphagia group (patients who developed dysphagia after stroke)...

Monitoring of processed EEG under anesthesia II
Horáček M.
Anest. intenziv. Med. 2022;33(3-4):153-160 | DOI: 10.36290/aim.2022.025
Processed EEG (pEEG) monitoring is currently used in four indications. The benefit has been demonstrated in the reduction of awakening and recovery times and in reducing the risk of accidental awareness during general anesthesia with the TIVA technique, while the indications for prevention of too deep anesthesia and personalization of anesthesia management are still questionable. Intuitively, it seems that personalized management of anesthesia using data not only from pEEG, but also from other basic monitors could improve results. The relevant studies are still ongoing. Spectrograms obtained by Fourier transformation of the raw EEG curve are suitable...

Anest. intenziv. Med. 2022;33(3-4):161-163 | DOI: 10.36290/aim.2022.024
Muscle relaxants are the most common cause of allergic reactions during the perioperative period. This reaction could be potentially life-threatening because of the intravenous route of administration of the drugs used for general anesthesia. We describe a case of rocuronium-induced anaphylactic shock in a 26-years-old woman with suspected extrauterine pregnancy successfully treated with sugammadex.
VV‑ECMO in organ donor after brain death - case report and review of the issue
Stadlerová B., Černá Pařízková R., Škola J., Škulec R., Pokorná E., Šustek P., Prudil L., Černý V.
Anest. intenziv. Med. 2022;33(3-4):164-169 | DOI: 10.36290/aim.2022.026
The lack of suitable organs is a global problem for transplant medicine. Donors with brain death represent the most common group of organ donors in developed countries and the Czech Republic. After brain death, organ dysfunction often develops, which can lead to organ damage and affect its viability for successful transplantation. Extracorporeal membrane oxygenation is an effective option of circulatory and lung support in the event of failure of normal procedures. The case report describes the clinical course of a 36-year-old donor with proven brain death who was connected to venous ECMO to bridge severe hypoxemia as a bridging technique for organ...

The article gives an overview of the use of curare from a poison of hunting arrows to its use in medicine as a muscle relaxant and a review of other substances that followed. People who have made a significant contribution to this history are listed. The development of muscle relaxant antidotes, the discovery of the nicotinic acetylcholine receptor, and the use and abuse of muscle relaxants outside of operating rooms are described.
