Multimodal analgesia is predicated on interactions of medicines whereby benefits of combination exceed the risks. Perioperative gabapentinoids have significant adverse effects. Gabapentinoids, when combined with opioids, confer even greater respiratory risk than opioids alone. Routine perioperative administration of gabapentinoids is not more recommended. New clinical trials to evaluate analgesic benefits of gabapentinoids on postoperative pain are very unlikely to provide any new evidence.

The article is a continuation of the series on the importance of fascia for regional anesthesia. This chapter attempts to align the anatomical structure of the lower limb with the requirements for regional anesthesia. The basic structure studied are deep fascia in the thigh area - fascia lata and deep myofascial structures. Fascia lata divides the muscles of the thigh into three compartments. 66 y.o. patient with end-stage chronic obstructive pulmonary disease was hospitalized in a standard ward, after abrupt onset of dyspnea a chest X-ray showed a pneumothorax. However, ultrasound examination before pleural drainage did not confirm pneumothorax and thus drainage was not performed. A computed tomography scan showed vanishing lung syndrome, explaining the X-ray finding. This case report points out vanishing lung syndrome and demonstrates complementarity of different imaging methods in diagnostic of pneumothorax in patient with complicated intrathoracic pathologies.

Shaken adult syndrome is a diagnosis related to the long-known shaken baby syndrome. However, the existence of this diagnosis in adults is debatable and only a minimal number of cases have been published worldwide so far.
