Anest. intenziv. Med. 2020;31(4):146-151 | DOI: 10.36290/aim.2020.033
Objective: Comparison of changes in ventricular function during sedation with midazolam and dexmedetomidine by magnetic resonance imaging (MRI), assess the feasibility of the implemented study protocol and determine the sample size of our randomized study. Design: A pilot randomized study. Setting: District hospital. Materials and methods: The study included participants who met the inclusion criteria. Enrollment was by the envelope method: 8 participants in each group. After signing the informed consent, cardiac MRI was performed before and after sedation. During the examination, the blood flow velocity through the mitral valve during the early (E...

Anest. intenziv. Med. 2020;31(4):152-156 | DOI: 10.36290/aim.2020.039
Objective: Evaluation of antibiotic prophylaxis in surgery. Design: Cross-sectional study. Setting: Regional healthcare facility. Material and methods: This study assessed patients (≥18 years old) who underwent a surgical operation in the defined period and agreed to participate in the study. Data were collected through the form for antibiotic prophylaxis perioperative record (identification and gender, information about surgical operation, choice of antibiotic, dose of antibiotic, time of administration and duration of it), and from the medical records. Collected data were compared with evidence-based medicine and internal hospital standards. The...

Our article after ten years: The practice of therapeutic mild hypothermia in cardiac arrest survivors in the Czech Republic
Škulec R., Truhlář A., Knor J., Šeblová J., Černý V.
Anest. intenziv. Med. 2020;31(4):165-171 | DOI: 10.36290/aim.2020.036
In 2010, the results of a national survey on the use of therapeutic mild hypothermia in patients successfully resuscitated from Cardiac arrest or sudden circulatory arrest hospitalized in intensive care units in the Czech Republic were published. This article reflects the results of the survey in the context of knowledge at the time of publication and from the perspective of current knowledge.

Recently, the therapeutic approach using the extracorporeal hemadsorption technologies attracted much attention. Effort to affect the patophysiological processess associated with critical illness represents an attractive concept, that penetrates into clinical practice. Here we present the critical opinion on current status of hemadsorption therapy in critical ill patients in the context of published evidence.
Insulin resistance, hyperglycemia and protein catabolism in the critically ill: looking for keys of the locked door
Bakalář B., Zajíček R., Duška F.
Anest. intenziv. Med. 2020;31(4):176-183 | DOI: 10.36290/aim.2020.032
Insulin resistance is a uniform reaction in critically ill patients. Its accompanying phenomena are hyperglycemia and protein catabolism, which are generally associated with deleterious effects on the body. The efforts to influence insulin resistance have so far been ineffective in critically ill patients. The aim of this work is to give an overview of the current state of knowledge about the causes and consequences of insulin resistance in critically ill patients and describe the current possibilities of influencing catabolic processes.

Anest. intenziv. Med. 2020;31(4):184-187 | DOI: 10.36290/aim.2020.025
The differential diagnosis of thrombocytopenia of critically ill patients with sepsis is often complicated and time-consuming. Apart from septic thrombocytopenia we have to consider also the immune thrombocytopenia. Immune thrombocytopenia may occur as a complication of servere acute infection especially in case of preexisting autoimmune or systemic disease. The leading symptoms are the decrease of platelet count and possible various haemorrhagic symptoms. In the acute phase the immunosuppressive therapy is indicated (corticosteroids) together with the complex intensive care treatment. We present a case report of suspect immune thrombocytopenia associated...

Hypoxia and hypercapnia - how do the chemoreceptors work?
Astapenko D., Černý V.
Anest. intenziv. Med. 2020;31(4):195-197 | DOI: 10.36290/aim.2020.041
Proper oxygen and carbon dioxide tension is one of the basic prerequisites for maintaining homeostasis. In the context of the macroorganism, mechanisms have been developed that allow both acute and long-term responses to respiratory gas imbalances. Acute reactions are mediated by chemoreceptors (glomus caroticum, large vessel receptor zones, and chemoreceptor centers in the brain) and primarily affect the respiratory pattern. Chemoreceptors work by transducing a chemical signal into an electrical one. Its genesis proceeds in several pathways, which lead to the closure of the potassium channel, the opening of the calcium channel and subsequently to...
