Objective: The aim of our study was to assess the management of hypotension after general anaesthesia induction (GAIH). Design: Multicenter questionnaire study. Setting: Eight different size anaesthesiology departments located in the Czech or Slovak Republics. Materials and methods: The respondents responded to three identification questions (workplace, length of practice, specialized competence) and ten GAIH management questions in our online questionnaire. Takotsubo cardiomyopathy (syndrome) is acute and reversible systolic dysfunction in the apical and middle areas combined with hypercontractility of the basal segment of the left ventricle. The clinical presentation of Takotsubo is similar to that of an acute coronary syndrome and myocardial infarction with ST elevation and cardiac enzyme rise. Coronarography is negative. Takotsubo cardiomyopathy affects especially postmenopausal women and the main trigger is severe psychological stress. Recently, several case reports described the onset of Takotsubo cardiomyopathy intraoperatively in patients undergoing thoracic spine surgery. We report the case of...

Background and goal of the study: Suboptimal visualisation of laryngeal structures could be associated with improper tracheal tube position or local airway trauma. The goal of this study was to compare the initial depth of tracheal tube insertion, rates of unintended endobronchial intubation, and postoperative patient discomfort after tracheal intubation using either videolaryngoscopy (GlideScope Titanium) or direct laryngoscopy (Macintosh blade). Type of study: Prospective, randomized, open study. Setting: Operating theatres of the University Hospital. Materials and methods: Ninety adult ASA I - III patients...

Anest. intenziv. Med. 2019;30(3-4):137-144
The paper is a continuation of the cycle of articles about the importance of structures the fascia planes for regional anesthesia. This article builds on the general part and deals with fascia planes and spaces in the neck area. The main neural vascular bundle passes between the surface and the deep fascia of the head towards the fuselage. Detailed knowledge of the fascial spaces makes it easy to understand the distribution of local anesthetics and minimizes the possible complications of unwanted Central blockades, vegetative and motor nerves.
De-escalation of antibiotic therapy as a part of care strategy about critically ill patientsIntesive Care Medicine - Review Article

Anest. intenziv. Med. 2019;30(3-4):145-152
One of the biggest problems of medicine is the extreme increase of bacterial resistance making the antibiotic treat-ment harder on the ICU as well as in the community. In order to maintain the efficacy of existing antibiotics, close interdisciplinary cooperation in diagnostics of infectious diseases along with an appropriate choice of antibiotics is crucial. Antibiotics must only be used for the treatment of bacterial infections, not as treatment of natural bacterial colonisations. Although de-escalation is one of the strategies of antibiotic stewardship, its role in intensive care is still unclear. For any modification in the antibiotic treatment,...

Chronic pain is not always manageable with standard treatment modalities. Moreover, physicians are now globally on the constant lookout for opioid sparing in chronic pain management due to the "opioid crisis", and neuromodulation treatment may by the possible answer. Neuromodulation is the process of inhibition, stimulation, modification or therapeutic alteration of activity in the central or peripheral nerve systems, with the use of electric current or drugs. The most important parts of neuromodulation for pain management are neurostimulation and intrathecal drug delivery. The use of neuromodulation has evidently seen growth over the course of the...
