Metamizole - 100 years since discovery

Curious history of a non-opioid analgesic metamizole is presented. Metamizole is a direct derivate of the first chemical analgesic drugs synthetized in the 19th century. During 100 years of its existence, metamizole was banned in many countries, while in others, including the Czech Republic gains popularity and is frequently used as a compound of multimodal treatment of postoperative pain.

The article describes the embryological and anatomical consequences of the development of the fascial system of the thorax in relation to the possibilities of regional anesthesia of the lumbar region. Using literary sources, schematics, ultrasound examinations, and computed tomography, we present the concept of four separate fascial compartments in the lumbar region. This concept can facilitate the understanding of blockades in the lumbar region and refine the indications and procedures

Albumin is still a widely studied protein of the human body. Historically, it has been infused as a volume replacement using its oncotic properties. In the field of fluid therapy in recent years, it has been compared with other types of solutions in various clinical indications and its safety was monitored. Sedation in the intensive care unit - part II. Depth of sedation monitoring using EEG signal analysis

Pharmacological sedation is one of the most common therapies in the intensive care unit (ICU). Current approach is minimalization of sedation and frequent monitoring of its level, using validated sedation scales. The depth of the anaesthesia and sedation monitoring based on the automated EEG analysis is used in anaesthesiology for more than two decades. The second part of the review is focused on the development of this technology and its possible application in the ICU.

The European Committee for Hyperbaric Medicine (ECHM) focuses its activities on supporting the improvement of the quality of care and safety in hyperbaric medicine. Donation after cardiac death: How to do it?

Implementation of donation after circulatory death (DCD) program has a potential to significantly increase the pool of transplantable organs. A typical DCD donor is a patient with unsurvivable brain injury, who does not fulfil formal criteria of brain death. The DCD implementation is supported by Czech legislature and guidelines are available. Yet, only few hospitals have implemented it. This educational paper summarises ethical, medical and logistical challenges related to DCD organ donation program.
Recommendations for the so‑called off‑label use of medicinal products in COVID-19 patients

During the epidemic of COVID-19, there often arises an urgent need for the so-called off-label use of medicinal products, i.e. use of a medicinal product that is not in accordance with its Summary of Product Characteristics (SPC). Most often, a medicinal product registered for a different indication is used. The goal of this paper is to provide the reader with an analysis of legal conditions under which the off-label use of medicinal products in COVID-19 patients is legally safe. If the provider of health services fulfils these conditions, they will not be held liable for immaterial harm that could potentially occur to the patient.

Neglected cause of anisocoria - unilateral mydriasis

Sudden anisocoria is an emerging neurological symptom. However, some causes are transient and benign. The authors present two cases of unilateral mydriasis, which evolved as an adverse local effect of nebulization treatment with ipratropium. Due to the frequency of administration of ipratropium in emergency and intensive care medicine, this may be a common but rarely noted complication. Consistent clinical neurological examination and comprehensive consideration of patient treatment can be helpful to avoid further unnecessary burdensome examinations.
