Surgically untreatable tracheal stenosis is a very serious condition that requires repeated interventions of the interventional bronchologist and has an uncertain prognosis. This case report describes the definitive solution of tracheal stenosis, following long-term unsuccessful dilatations and stenting, by the insertion of the Montgomery T tube. The procedure requires close cooperation between the interventional bronchologist and the anaesthetist, using a combination of rigid and flexible bronchoscopes and high-frequency jet ventilation. In this patient, the insertion of the Montgomery T tube provided a significant improvement in the quality of life,...

Objectives: To determine the anaesthesia practice for early postpartum procedures in the Czech Republic (CZE) and Slovakia (SK). Design: International, prospective, observational. Departments: 105 departments providing obstetric anaesthesia in CZE and SK. Methods: With Ethical Committee approval we aimed to enrol all the 149 obstetric departments in CZE and SK and to monitor every case of anaesthetic care in the peripartum period during November 2015. The data were recorded into CLADE-IS database through a Case Report Form consisting of two parts (Demography 2014 and Case Report). The database was analysed descriptively...

The ultrasound machine has become a practically irreplaceable aid for safe and successful regional anaesthesia. Introduction of ultrasonic assistance into regional anaesthesia represents a revolutionary change comparable to that of the introduction of electro-stimulating methods to locate nerves. Direct visualization of needles and target organs and in particular monitoring of the distribution of the local anaesthetic in real time fundamentally influence normal clinical practice. Due to the ultrasound navigation, there a reduction in the risk of serious complications had been achieved. There has also been a reduction in the doses of local anaesthetics...

Anest. intenziv. Med. 2019;30(2):86-89 | DOI: 10.36290/aim.2019.027

Mucormycosis is a very serious, invasive fungal infection, occurring frequently in patients with a modified immune response. Mortality in cases of a generalized form of mucormycosis can reach 96 %. The principles of successful treatment of this disease are early diagnosis and aggressive antimycotic and/or surgical therapy. In this case report we point out the clinical importance of correct timing of therapy, even though even early and appropriate management does not always guarantee survival.

Intensive care medicine is a complex, demanding and rapidly developing field combining the need for good knowledge of anatomy, physiology, biochemistry or pharmacology with scientific evidence coming from clinical studies and, last but not least, personal, clinical experience. Especially for younger doctors, it may be difficult to comprehend the amount of information available today and to give them the appropriate weight to avoid treatment based on "fashion trends" or, vice versa, a nihilistic denial of any progress. In this special article we tried to offer an overview of freely available online resources and discussion forums which can help our...

Malnutrition is an important negative factor impacting on postoperative morbidity and mortality in surgical patients. In addition, starvation raises risk of complications in the perioperative period after major surgery. Minimisation of the duration of starvation and protein-caloric deficiency requires careful and permanent monitoring of nutritional care with early response in the perioperative period. It includes modification of diet, sipping and eventually administration of enteral or parenteral nutrition. An up-to-date perioperative care model is the concept of ERAS (Enhanced Recovery After Surgery), where nutritional elements play a fundamental...
