Management of the critical patient in the undergraduate teaching of medicine is an important part of first aid, intensive care medicine and anaesthesiology. The aim of this paper is to present an overview of the interactive learning opportunities which are offered to students at the Medical Faculty of Masaryk University (MF MU): the use of the virtual patient (interactive algorithms, clinical scenarios), the standardised patient, skills training on patient models, simulators with an embedded computer model or videoconference streaming from operating theatres. These teaching methods have become part of undergraduate teaching of medicine, dentistry and...

Objective:Retrospective evaluation of a sample of 21 neonates and children treated with ECMO. Design:Observational, retrospective with prospective data collection. Setting:Paediatric and neonatal intensive care unit of a teaching hospital. Materials and methods:Between October 2010 and present, extracorporeal membrane oxygenation support was initiated in 21 patients based on meeting the indication criteria (12 neonates, 9 children). According to the etiology and severity of the cardiorespiratory failure, veno-arterial (V-A) ECMO was indicated in 57% of the patients (83% of the neonates, 22% of...

Vasopressin has an essential role in the regulation of the vasomotor tone. Its insufficient level plays a substantial role in the pathophysiology of refractory shock associated with sepsis, haemorrhage and extracorporeal circulation. Its role in the control of arterial pressure is emphasised at times when other systems are blocked, not functional, or incapable of maintaining blood pressure - for example in septic shock or in refractory vasoparalysis. A low dose of vasopressin may contribute to normalisation of the vascular tone and perfusion of vital organs. The indications for vasopressin administration include septic shock with a need for high-dose...

Haemodynamic assessment of critically ill patients is an essential diagnostic step for verification of sufficient organ perfusion and oxygen delivery and, moreover, one of the most important tasks of critical care medicine. During the last decades, cardiac output measurement has become a cornerstone topic with simultaneous development of various methods for its continuous or intermittent monitoring. Echocardiography has been recognized as a non-invasive, nevertheless accurate method for the cardiac output measurement. It is especially important to understand the basic principles of ultrasound in order to use echocardiography properly, and to realize...
