Acute aortic dissection is a life-threatening condition with high mortality rate. One of the many risk factors of this disease is the physiological pregnancy. It is necessary to make early diagnosis and start adequate treatment according to the needs of both the mother and the phoetus. We present a case of a pregnant woman at 33 weeks of gestation with type A dissection, and the management of general anaesthesia. Following a Caesarian Section, the ascending aorta was repaired.

Marfan syndrome is an inherited disease affecting mainly the cardiovascular, respiratory and muskuloskeletal systems. Aortic dilatation and dissection is the major cardiovascular complication associated with high mortality. Pregnancy and peripartum are a critical period accelerating the risk of acute aortic dissection. Peripartum management of patients with Marfan syndrome represents a special complex multidisciplinary issue. Due to the rare diagnosis and small patient population, the clinical experience is limited even in specialized centers. A case report and a literature research focused on theophylline intoxication is presented. There is a high risk of theophyllin overdosing due to its narrow therapeutic range. The intoxication can cause a life threatening state requiring adequate treatment. Gastrointestinal and cardiovascular symptoms are the most frequent. Tachyarrhythmias respond to betablockers: titration of intravenous esmolol may be advantageous in a haemodynamically unstable patient. Therapy is directed by symptom severity and theophyline plasma level.
