Objective:Surgical securing of a patent airway is a life-saving procedure in the scenarios where other methods have failed. The aim of this study was to evaluate bougie-assisted cricothyrotomy (BACT) on a porcine model. Design:Interventional pilot study performed on a porcine model. Materials and methods:We performed bougie-assisted cricothyrotomy on 10 mini-pigs under general anaesthesia. The following parameters were measured: procedure duration, success rate and number of attempts needed. Other evaluated parameters included anatomical location of the incision, trauma to the posterior tracheal wall and other...

This case report illustrates the use of continuous caudal epidural blockade in postoperative analgesia of an extremely premature baby. A patient with body weight of 550 g was treated for a severe form of necrotizing enterocolitis. Systemic analgesia was no longer sufficient, and therefore it was combined with continuous caudal epidural analgesia by a 1F (28G) catheter. Continuous levobupivacaine 0.25% was administered at 0.2 mg/kg/h. The quality of analgesia was evaluated by the Comfort Neo score with a satisfactory result.

Postdural puncture headache (PDPH) is a relatively common iatrogenic complication (about 1% of all neuroaxial punctures) in obstetrics. The reasons for higher incidence in pregnant women compared to the general patient population are mainly the physiological and pathophysiological differences in pregnant women, but also the increasing number of neuroaxial blockades used. Because untreated postdural puncture syndrome can result in fatal complications, the basic principle of treatment is a complex multimodal approach consisting of all available conservative and invasive therapies, adequate communication and psychological support of the patient. The aim...

Beta-1-adrenergic blocking agents are widely used in patients with ischaemic heart disease and patients with heart failure. Anaesthesiologists are in daily contact with these drugs as they are part of medication in anaesthetised patients undergoing surgical procedures. Long-term mortality benefit effect of these drugs has been proven. Esmolol as a representative of ultrashort-acting beta-blockers can be safely used to control septic tachycardia and promote myocardial protection in this situation. According to published data it can be used even if a patient has concomitant catecholamine infusion. In the peri-operative period, esmolol is indicated for...

We describe a case of a patient with traumatic rupture of the spleen and liver, which was complicated by candid sepsis. The infection was endogenous, originating from the bowel. Empirical therapy with echinocandin was started. The diagnosis was confirmed by finding Candida krusei and Candida glabrata in abdominal tube secretion culture and also in blood culture. The sepsis was successfully treated. The patient died because of complications not related to the candid sepsis.

Prolonged coagulation tests (INR and APTT) are a common occurrence in ITU patients. Thromboelastography (TEG) and rotational tromboelastometry (ROTEM) have been used since the last century in various fields of medicine. Unlike conventional laboratory tests they assess the function of other blood components especially the platelets. In this article we highlight the importance of TEG/ROTEM in intensive medicine. We present cases where this methodology proved to be more reliable than conventional tests both pre-operatively, postoperatively or in the differential diagnosis of bleeding events. Using this methodology in these and many other cases we were...
Current techniques of continuous non-invasive monitoring of arterial blood pressureIntesive Care Medicine - Review Article

Blood pressure is one of the most important parameters monitored in anaesthesia and intensive care medicine. Two possibilities of automatic blood pressure monitoring are at hand in contemporary practice: intermittent measurements obtained via an oscillometric cuff or continuous monitoring based on arterial catheterisation. In recent years novel technologies have been developed enabling continuous non-invasive monitoring of blood pressure. This review article describes the two most developed of these technologies: the volume clamp method and appalanation tonometry. Devices based on these principles might in near future help to significantly improve...
