Anest. intenziv. Med. 2011;22(2):96-102
Hemodynamic optimization of high-risk surgical patients has been increasingly used in the peri-operative setting. The benefit of this approach for the morbidity and mortality reduction in the postoperative period has been proved by many studies. In this article we summarize and comment on the articles published on perioperative goal-directed therapy.

Anest. intenziv. Med. 2011;22(2):103-114
Background:Tranexamic acid can reduce bleeding in patients undergoing elective surgery. We assessed the effects of early administration of a short course of tranexamic acid on death, vascular occlusive events, and the receipt of blood transfusion in trauma patients. Methods:This randomised controlled trial was undertaken in 274 hospitals in 40 countries. 20,211 adult trauma patients with, or at risk of, significant bleeding were randomly assigned within 8 h of injury to either tranexamic acid (loading dose 1 g over 10 min then infusion of 1 g over 8 h) or matching placebo. Randomisation was balanced by centre, with an...

Anest. intenziv. Med. 2011;22(2):115-123
About 500,000 people suffer a sudden cardiac arrest in Europe every year but their treatment has not been satisfactory for years. The main issues in the Guidelines for cardiopulmonary resuscitation launched by the European Resuscitation Council in October 2010 are summarized in the article. The changes in the new recommendations are based on recent scientific evidence in resuscitation medicine and an effort to make them simpler compared to the last 2005 Guidelines. Chest compression remains the most important action in both basic and advanced life support. The victim's chest must be pushed hard into the depth of at least 5 centimetres with a frequency...
Potential impact of criminal charges and claims of inadequate health care on the physicians' performanceEthics and Medicine Law

Anest. intenziv. Med. 2011;22(2):124-127
In recent years an increasing number of criminal charges and claims of inadequate health care provided by doctors have been observed. The driver of this increased number of claims and criminal charges is not usually based on poor quality of work, but is a result of targeted negative campaigns directed to strengthening the legal awareness of a patient by encouraging patients to lodge complaints and criminal claims against doctors in order to achieve compensation for alleged injury. The article gives some advice about how physicians should behave when facing allegations by the legal system.
