Objective:To assess the incidence and risk factors for postoperative sore throat. Design:A prospective, observational, double-blind study. Setting:Departments and ICUs of Surgery, Trauma, Plastic and Aesthetic Surgery, Urology and Gynaecology, University Hospital Olomouc. Materials and methods:The study comprised of 1,942 patients after elective surgery who were inquired about sore throat and other peri-operative complaints on the 1st postoperative day. If sore throat or hoarseness were present on the 1st postoperative day, the patients were re-examined on the 3rd postoperative day....

Objective:Alpha-2 sympatho-mimetic drugs are used to prevent adverse peri-operative reactions. The aim of this study was to compare premedication with dexmedetomidine against other currently used agents. Design:Prospective, randomised, double-blinded clinical study. Setting:University Hospital. Materials and methods:After ethic committee approval and patients' consent, premedication with atropine 0.5 mg and one of the following agents was administered in a randomised and blind fashion to the deltoid muscle 15 min before induction to anaesthesia: dexmedetomidine 1.0 µg . kg-1...

Objective:To elucidate the functional anatomy of the brachial plexus, its relationship to the surrounding structures and their influence on the spread of local anaesthetics. Furthermore, to compare the clinical experience and the role of functional anatomy on the extent and quality of various techniques of brachial plexus blockade. Design:Prospective, human study. Setting:Departments of Anaesthesia and Radiology (CT suite). Materials and methods:Diluted (1 : 5) contrast agent (Iomeron 400, Bracco Imaging) was injected via the interscalene, infraclavicular and axillary routes to simulate brachial...

Objective:Acute liver failure (ALF) is a relatively sparse disease with a very poor prognosis. Its onset is accompanied by haemodynamic instability. The aim of our study was to evaluate the influence of fractioned plasmatic separation and adsorption (FPSA) on the haemodynamics using an ALF experimental model in large laboratory animals, comparing the results of the experimental and control groups. Design:Experimental study. Setting:Department of Anaesthesiology and Resuscitation, Central Military Hospital, Prague. Materials and methods:Our experiment included 21 laboratory pigs ( 25-45 kg ) with...

Objective:Prometheus, a device based on modified fractionated plasma separation and adsorption, is used in the therapy of acute liver failure as a bridging to liver transplantation. The therapeutic effect of Prometheus is not limited just to the elimination of terminal metabolites, therefore the aim of the study was to identify the effect of this method on the levels of cytokines and inflammation and liver regeneration markers. The effect on encephalopathy, haemodynamics and time on the waiting list for liver transplantation was also observed. Materials and methods:Total 11 patients with acute liver failure were investigated....

Acute liver failure is a rare but potentially fatal condition with mortality reaching 80-90 %. Dialysis, haemoperfusion and countinuous renal replacement therapy are currently the methods of choice in patients with this critical condition.Prometheus has been recently introduced in our PICU. This liver replacement device is designed to eliminate toxins produced during the liver failure state. So far two critically ill children - aged 7 years and 15 months - have been treated with Prometheus. Although unfortunately both the patients died of brain oedema, we witnessed a good elimination efficiency of the system. Ideal timing of the initiation of the...
