Robot-assisted laparoscopic surgery is a new and rapidly developing technology of the last decade. The surgeon takes advantage of the 3D surgical field imaging and manipulation scope beyond the capability of the current laparoscopic instruments. An uncomplicated course of robotic surgery significantly shortens the length of hospital stay compared to standard open surgery. On the other hand a robotic surgical system carries high acquisition and operating costs.It remains to be seen whether robot-assisted surgical procedures are safe and minimal-invasive even in patients with multiple comorbidities.

Objective:To determine the outcomes of paediatric intensive care in the Czech Republic in 2007. Design:Retrospective, multi-centre study. Setting:Intensive care units of tertiary paediatric centres. Materials and methods:We evaluated the 2007 PICU patient data - PICU length of stay, days on mechanical ventilation, invasive procedures (CVCs, arterial lines, ICP measurements and CRRT) and outcomes. Results:Of the total number of 5287 patients admitted to the 15 tertiary paediatric intensive care units (PICU), 134 patients died (mortality rate 2.5%).

Even though High-Frequency Ventilation (HFV) meets criteria defining so called protective ventilatory strategy, HFV is not widely used in the clinical practice. Early and appropriate antibacterial therapy is a principal intervention in septic patients. Adequate fluid resuscitation in the hyperdynamic stage of sepsis increases renal perfusion and via increased capillary permeability leads to a leak into the third space. The increased clearance and volume of distribution may result in low plasmatic concentration of antimicrobial drugs. A national task force representing clinicians (haematologists, oncologists, microbiologists and intensivists), specialist medical societies and working groups prepared evidence-based guidelines for the treatment of established fungal infections - invasive candidiasis - in adult haematology and ICU patients. These guidelines updated those published in the Czech Republic in 2003/2004. The quality of the clinical trials was assessed using the evidence criteria of the Infectious Diseases Society of America (IDSA) and the updated EORTC/MSG Consensus Group conclusions were used for definitions of invasive fungal disease.
