Anaesthesia options in carotid endarterectomyAnaesthesiology - Original Paper
Z. Mrozek, J. Koutná, O. Marek
Anest. intenziv. Med. 2007;18(4):202-209
Objective:Carotid endarterectomy is a procedure used for the treatment of partial carotid obstruction. Until recently it was performed under general anaesthesia (GA) but recently cervical plexus block (CB) has been used as an alternative. It enables monitoring of the level of consciousness and motor function of the patient during the surgery and therefore allows early diagnosis and intervention in cerebral hypoxia. The aim of the survey was to compare the two anaesthesia methods intra-operatively and in the early postoperative period. Setting:Department of anaesthesiology and intensive care, University Hospital. Materials...
Influence of low dose ketamine on anaesthesia for bronchoscopic proceduresAnaesthesiology - Original Paper
O. Marek, M. Adamus, Z. Mrozek
Anest. intenziv. Med. 2007;18(4):210-215
Objective:To investigate an optimal anaesthesia regimen for invasive bronchoscopy in patients with a various degree of perioperative risk. Design:Clinical prospective randomized study. Setting:Department of Anaesthesiology and Critical Care, University Hospital. Materials and methods:160 patients with tracheal stenosis were treated with laser therapy and insertion of a tracheal stent via a rigid bronchoscope. A combination of midazolam, ketamine, propofol and sufentanil was used for anaesthesia without neuromuscular blockade. The patients were randomised to intravenous ketamine 0.30 mg/kg group...
Single-shot paravertebral block in children - the initial experienceAnaesthesiology - Original Paper
E. Berta, J. Špaňhel, O. Šmakal, T. Gabrhelík, V. Smolka, B. Bertová, H. Špaňhelová, H. Fiala
Anest. intenziv. Med. 2007;18(4):216-220
Objective:To study the effectiveness of a single-shot paravertebral block in children following major urologic surgery. Both multiple-level and continuous paravertebral blocks had already been successfully used for postoperative analgesia in children. Design:Prospective, observational, partly observer-blinded trial. Setting:Department of Anaesthesia and Intensive Care, University Hospital. Materials and methods:A paravertebral block was performed at Th10-12 level in 8 children (median age 9.8 months [range 3.2-17.1]) undergoing renal surgery during the preparatory phase before a pilot study. At...
Notes on the pathophysiology of postoperative deliriumAnaesthesiology - Comprehensive Report
M. Hála
Anest. intenziv. Med. 2007;18(4):221-226
Postoperative delirium represents a serious complication after major surgery. Patients suffer from anxiety, hallucinations and delusions and have higher postoperative morbidity and mortality. A long-term cognitive impairment is its common consequence. Delirium thus represents a nursing, therapeutic as well as economic problem. In this review article the basic characteristics and incidence of postoperative delirium are summarized, and hypotheses of its development published so far are presented.

Non-invasive optical methods for microcirculatory assessment in experiment and in clinical practice: Orthogonal polarization spectral (OPS) imaging and Sidestream dark-field (SDF) imagingIntensive Care Medicine - Comprehensive Report
Z. Turek, R. Pařízková, V. Černý
Anest. intenziv. Med. 2007;18(4):231-235
Microcirculation plays an essential role in the interaction between the blood and target tissues. Despite its evident importance in the pathophysiology and development of numerous acute and chronic diseases such as hypertension, diabetes or sepsis, currently available methods allowing direct visualization of the microcirculatory network and its assessment in clinical practice are very limited. Orthogonal polarization spectral (OPS) imaging and its improved successor Sidestream dark-field (SDF) imaging are relatively new optical non-invasive technologies incorporated in a hand-held examination probe for visualization of the mucosal surface microcirculation...
Non-heart-beating donorsIntensive Care Medicine - Comprehensive Report

Anest. intenziv. Med. 2007;18(4):236-240
Since the 1990s the continuous quest for new organ resources for transplantation has re-evoked the interest in non-heart-beating donors (NHBD). The method was pioneered in the Netherlands and nowadays NHBDs are accepted in a wide range of countries. Since 2002 the Czech Republic has joined these countries in accepting this group of donors. The basic medical problem related to NHBD is the duration of the warm ischaemic time and the ischaemic-reperfusion syndrome. NHBDs remain a very specific group even from the ethical and legal aspects. Legislation of individual countries varies as much as the protocols of the individual transplantation centres. The...
