Anest. intenziv. Med. 2006;17(5):235-240
Objective:Postoperative delirium (POD) is a common complication after major surgery. Our aim was to prove the hypothesis that systemic inflammatory response syndrome (SIRS) contributes to the development of POD. SIRS is a complex network of mutually interacting processes where one of the terminal tissue effectors is COX-2. Design:Prospective open-label study. Setting:Center for Cardiac Surgery and Transplantation. Materials and Methods:86 consecutive patients all over 65 years of age with coronary artery disease were enrolled. All of them were indicated for open-heart surgery (with or without...
Duration of rocuronium-induced neuromuscular block predicted by its dose and onset time: a prospective studyAnaesthesiology - Original Paper

Anest. intenziv. Med. 2006;17(5):241-245
Objective:Following a bolus dose of rocuronium, the average duration of neuromuscular block until 25% recovery (= clinical duration) can be predicted by the dose-duration relationship. In the present study we investigated whether the predictive power could be improved by the addition of another independent variable - onset time. Design:Clinical, prospective, randomized, controlled study. Settings:Department of Anaesthesiology and Intensive Care, University Hospital, Olomouc. Materials and Methods:Following local ethics committee approval and obtaining informed consent, 350 patients scheduled for...
Fuzzy logic in anaesthesiology as a way of thinking and a tool for practical applicationsAnaesthesiology - Comprehensive Report

Uncertainty and indeterminacy are typical of expert knowledge in humanities. Data processed by an expert (e. g. a medical doctor) are often imprecise, uncertain, and indeterminate. It is, however, these data according to which the expert has to make his/her decisions. Processing of indeterminate and uncertain data is the subject of fuzzy logic, which deals with fuzzy sets. In a broad variety of situations and tasks for which no precise mathematical models are available, an expert is able to formulate his/her knowledge and problem solving strategy in a natural language by means of so-called"if-then"rules. Rule-based fuzzy systems, which are based on...

Anest. intenziv. Med. 2006;17(5):251-260
Objective:The purpose of the study was to obtain information about the attitudes of doctors working on ICUs in the Czech Republic to limiting therapy and end of life decisions (EOLD). Design:Prospective, multi-centre, observational study. Setting:ICUs in tertiary care hospitals. Materials and Methods:Questionnaires were posted to members of the Czech Society of Anaesthesiology and Intensive Care on their attitude to limiting therapy in patients admitted to ICUs. Results:870 posted questionnaires yielded a response rate of 26%. 90% of the physicians considered limiting therapy in...
Limitation of therapy in patients on ICU - a one-day national studyIntensive Care Medicine - Original Paper
R. Pařízková, V. Černý, K. Cvachovec, I. Novák, V. ©rámek, D. Nalos
Anest. intenziv. Med. 2006;17(5):261-265
Objective:The purpose of the study was to obtain information about patients experiencing any form of limitation of therapy (withdrawal - WD, or withholding - WH of therapy) and the way it is performed on the ICU. Design:Prospective, multi-centre, one-day prevalence study. Setting:Intensive Care units. Materials and Methods:An analysis of the posted questionnaires regarding ICU patients with limitations of therapy on the day of the study (16/3/2005) was performed. Information about the type of ICU, the patients with limitation of therapy and its form was obtained. Attention was paid to the documentation...
Extreme hypoxic lactaemia in patients on critical care - prognosis, therapeutic options and their limitationsIntensive Care Medicine - Original Paper
M. Kolář, A. Součková, Z. Stach, J. Valenta
Anest. intenziv. Med. 2006;17(5):266-270
Objective:The determination of prognostic factors and evaluation of therapeutic options in patients with tissue (cellular) hypoxia presented as high lactate levels. Type of study:Retrospective study. Setting:Department of Anaesthesiology and Intensive Care Medicine, teaching hospital. Material and Methods:Inclusion criteria were serum lactate > 8 mmol/L at any point during the first 24 hours after admission and the presence of tissue hypoxia (PaO₂ < 8.8 kPa and/or SpO₂ < 90%, PvO₂ < 4.7 kPa) in association with conditions inevitably associated with hypoxia (circulatory...
