Objective:Our aim was to introduce single-shot caudal epidural block with bupivacaine and clonidin as a standard analgesic component of general anaesthesia in children undergoing hypospadia repair. We focused on analgesia after surgery and its side-effects. Design:Open prospective observation. Setting:Department of Anaesthesia and Intensive Care and Department of Plastic and Aestetic Surgery, St. Ann's University Hospital Brno, Czech Republic. Material and Method:66 boys were included into the study. Bupivacaine (2 mg/kg) and clonidin (2.5 mcg/kg) with saline were injected into the caudal space...
Breathing Filters and Heat and Moisture Exchangers: Current Thinking Exposed!Anaesthesiology - Comprehensive Report
J. Ewart
Anest. intenziv. Med. 2005;16(2):94-100
Breathing filters are becoming a standard tool used in the hospital environment to combat the possibility of the spread of nosocomial infection in ventilated patients around the world. This review includes an update on current thinking concerning breathing filters, a re-evaluation of currently used terminology and correct validation and use of product, as viewed from a manufacturers perspective. Perhaps the two main issues this presentation attempts to draw attention to includes challenging the traditional models describing the way breathing filters function as well as discussing the prefered test protocol by which breathing filters can be evaluated...

Pulmonary endarterectomy (PEA) is a well-respected method of therapy of the pulmonary hypertension caused by chronic thromboembolic disease. It is performed only at a small number of world-wide centres. Successfully operated patients have a good long-lasting prognosis and highly improved quality of their lives. The authors present two case reports of the first patients on whom the PEA was performed in the General University Hospital in Prague. In the discussion the authors mention the cerebral protection during a deep hypothermic cardiac arrest and prevention of reperfusion lung injury as a key points of PEA influencing the choice of an anaesthesia...

Objective:Survey of the level of respiratory care at Czech ICUs in 2004. Design:Prospective multicentre one-day observational study using a questionnaire. Material and Methods:Evaluation of responses to a questionnaire sent to members of Intensive Care Section of the Czech Society of Anaesthesiology and Intensive Care Medicine. Results:Data from 70 ICUs with 570 beds were obtained. The mean length of ICU stay was 7.8 days. 407 patients were currently hospitalized (mean age 58 years). Out of 535 ventilators 60% were equipped with respiratory mechanics chart. Bronchoscopy is performed by intensivists...

We are reporting a case of the acute intoxication by sodium fluosilicate (Na2SiF6) in a 27-year old patient after using 3-4 teaspoons of the white powder of the Na2SiF6. One and half hour after consuming the powder the patient was admitted to our department, where the organ dysfunction occurred within 6 hours and resulted in death. The dose of poison used by the patient corresponds with the data referred in literature as the lethal dose (Na2SiF6 over 5-10 grams or 70-140 miligrams/kg of body weight orally).
