Over six decades ago, our medical pioneers attemped to manipulate the heart's rythm using technology, to reach the goal for this "simple pump" to fulfil its primary physiological function. The fruits of their labour borne neither easily, nor immediately, but owing to their commitment, we could execute our medical duties using safe and state-of-the-art technology for the past several years.

The article is another chapter in the series describing the importance of the fascias for regional anaesthesia. This chapter deals with the fascial planes of the nerves supplying the upper limb. The main vascular and nerve bundles for the cervical region run on the neck between deep and superficial fascias. There are several separate fascial planes under the deep cervical fascia, in which the individual nerves and the brachial plexus take place. Negative pressure pulmonary edema (NPPE) is a rare anaesthetic complication, which is not yet mentioned in Czech literature. NPPE is a type of a non-cardiac pulmonary edema, caused by negative intrathoracic pressure following airway obstruction. Young healthy males, able to develop extreme inspiratory effort, are most prone to this complication. NPPE occurs in 0.1 % of all general anaesthetics but is very likely under-diagnosed. Although the treatment is relatively easy and the prognosis is great in most cases, any delay or misdiagnosis can lead to hypoxic damage or death.
