The first combined heart-lung transplantation in the Czech Republic was successfully performed on November 22nd, 2007. The patient was a 49-year woman who suffered from congenital heart disease complicated by severe pulmonary hypertension which excluded the possibility of heart transplantation only. The uncomplicated surgery took 8 hours and her ICU stay was 12 days. The patient was discharged from the hospital on January 11th, 2008 in a very good condition. Presently she does not complain of any serious problems and is under regular follow-up.

The article shows the potential of the Power M-mode in TCD examination of seriously ill patients. The authors analyze their experience with this relatively new method through case reports and therewith they show the pitfalls, advantages and disadvantages of the method. TCD is a non-invasive bedside examination that can significantly shorten the time of brain death diagnosis and allow earlier activation of the organ salvage programme within the current legal constraints.

ALI/ARDS stems from the inflammatory response to a severe pulmonary or extrapulmonary injury. An intervention in the pro-inflammatory imbalance seems to be a logical life-saving step in addition to the general principles of ARDS management. The biomolecular background of corticosteroid treatment of patients with ARDS could be the cytoplasmatic glucocorticoid receptor-α. The review summarizes the fundamentals of carbon dioxide homeostasis. The impact of hypercapnia on the vital organs, organ systems, microcirculation and cellular function are described. Hypercapnia is discussed in the context of protective ventilation and respiratory acidosis. The current recommendations in protective ventilation of the critically ill and the contraindications to permissive hypercapnia are listed.
