The article deals with conclusions the American-European consensus Conferences on ALI/ARDS and VALI and their impacton current protocol of mechanical ventilation. Re-targeting of goals of mechanical ventilation, which give priority toavoidance of lung damage with inappropriate ventilation pattern leads to introduction of lung protective ventilatory strategy. It is based on Lachmann's concept of "open the lung and keep it open". Barotrauma, volotrauma and atelectrauma arerecognized as main determinants for lung damage. The causative relation between lung damage due to mechanicalventilation and its deleterious effect on extrapulmonary organs leading to...

Severe respiratory failure featuring profound hypoxemia resistant to conventional therapy may occur in somepatients. "Rescue therapy procedures" have been considered as treatment options mostly in experimental setting or instrictly defined and limited cohorts of patients. Selected experimental and clinical trials evaluating therapy with inhalednitric oxide, surfactant treatment, liquid ventilation, high frequency oscillation, extracorporeal lung assisting devices andprone position are discussed.Based on currently available results,prone...

Intrapulmonary Conditions and Ventilatory Parameters in Conventional and High FrequencyVentilationArticles

K. Roubík, J. Pachl

Anest. intenziv. Med. 2003;14(4):181-187

The article deals with differences between the conventional mechanical ventilation and a high-frequency ventilation. First, the different physical principles of gas flow are described. Then, effects of the mechanical properties of the respiratorysystem upon intrapulmonary conditions are studied in both ventilatory regimens. Differences between the parameterscharacterising each ventilatory regimen are explained. Finally, the importance of the ventilatory parameters for a safe andefficient application of both conventional and high-frequency ventilations is examined.
