Sugammadex is a novel drug for reversal of rocuronium or vecuronium-induced neuromuscular blockade. Compared to neostigmine, its mechanism of action (encapsulation) is not influenced by cholinesterase activity. In this article, the authors summarize the current indications of sugammadex, its advantages over neostigmine and its value in specific situations during anaesthesia.

Despite significant impovements in surgical management and perioperative techniques, adverse neurological outcomes still remain a serious postoperative complication in patients undergoing cardiac surgery with an estimated incidence that ranges from 2% to 5% in most centers. Cerebral oximetry has been postulated as an emerging tool for real-time detection of clinical scenarios that can potentially predispose to cerebral injuries. Despite recent advances in near-infrared spectroscopy and an increasing availability of clinical data, the application of cerebral oximetry has not been as rapid as expected due to several reasons such as: a) disparities between...
New role and rennaisance of branched chain aminoacids in intensive careIntesive Care Medicine - Review Article

Anest. intenziv. Med. 2012;23(5):253-258
The branched chain amino acids valine, leucine and isoleucine are essential nutrients with more than nutritional effects. They are important mediators and structural components in proteosynthesis and they regulate hormonal functions. Leucine is a precursor in cholesterol synthesis and as such plays an important role in muscle tissue growth and in sarcopenia suppression. Older and recent data are reviewed and support wider utilization of branched chain amino acids in intensive care.

This review analyses studies reporting preoperative risk factors for postoperative morbidity and mortality in patients with perforated peptic ulcer in the literature in the last 10 years. There exists a number of factors clearly associated with increased morbidity and mortality in this illness: advanced age, co-existing diseases, malnutrition, shock on admission and delayed surgery. Because some of the factors are predetermined, reduction in mortality is difficult to achieve. On the other hand detection of risk factors allows to stratify patients and to reduce the incidence of postoperative complications. Some scoring systems exist to determine the...
