Anest. intenziv. Med. 2010;21(6):311-316
Objective:The aim of the study was comparing patient satisfaction and pain after general vs. regional anaesthesia in total hip and knee replacement surgery. Design:A prospective, comparative study. Setting:Department of Anesthesiology and Resuscitation, Regional Hospital. Materials and methods:All the patients who underwent total hip or knee replacement in general (GA) vs. combined epidural/spinal anaesthesia (EA/SA) in a one-year period were included in the study. The patients evaluated their satisfaction and pain relief at 6, 24 and 48 hours after surgery. Results:We evaluated...

Anest. intenziv. Med. 2010;21(6):317-323
Objective:To compare the extent of therapeutic mild hypothermia (TH) usage in cardiac arrest survivors in the Czech Republic during the last years, to assess the impact of the guidelines on TH usage and to analyze the reasons of its non-use. Design:National questionnaire survey. Setting:Emergency Medical Service, Department of Anaesthesiology and Intensive Care Medicine. Materials and methods:After completing a list of all non-surgical intensive care units (ICU) in the Czech Republic, their lead physicians were sent a structured questionnaire in December 2006 (S2006) and October...

Anest. intenziv. Med. 2010;21(6):324-330
Objective:The main goal of this work was trying to replace conventional ventilation using an original, small internal diameter, long-cuffed endotracheal (ET) tube with a high flow resistance, by high-frequency jet ventilation applied via a special catheter (C-HFJV).The authors describe the use of high-frequency jet ventilation applied via a catheter for a translaryngeal (retrograde) tracheostomy by Fantoni (TLT). Type of study:Prospective observational study. Setting:Department of Anaesthesia and Intensive Medicine. Materials and methods:The authors were using the following...
Dynamics of the neurofilament pNF-H as a predictor of mortality in children with brain injuryIntensive Care Medicine - Original Paper

Anest. intenziv. Med. 2010;21(6):331-336
Objective:The aim of the study was to determine the importance of increased levels of a high-molecular weight form of neuro-filament subunit (pNF-H) in the serum of children with brain injury, as a predictor of mortality. Design:Prospective observational study. Setting:Department of Anaesthesiology and Intensive Care Medicine, University Hospital. Materials and methods: Total 50 children aged 0-19 years hospitalized between June 2007 and September 2009 with severe brain injury were examined. Serum samples were collected on admission and each following day for the first 6 days of hospitalization. Statistical...

Necrotizing pneumonia caused by Staphylococcus aureus producing Pantone-Valentine leukocidin (PVL) is a rare but life-threatening disease. Early diagnosis of staphylococcal etiology is necessary for successful treatment. Linezolid is the first choice antibiotic that should be preferred to beta-lactams and vancomycin in this indication. We report a case of 29-year-old man who was admitted for severe septic shock and incipient pneumonia and died 8 hours after admission despite intensive care. PVL-producing Staphylococcus aureus was isolated from the blood cultures.

Sepsis and septic shock are the most frequent causes of death in intensive care units. Thrombocytopenia and/or platelet function impairment are common parts of the multiple organ dysfunction syndrome. However, thrombocytes are not mere bystanders as documented in the present literature. Platelets, primary actors in haemostasis and thrombin generation during endothelial integrity disruption, similarly to leukocytes, maintain important functions of innate defense mechanisms of the host. These cellular fragments express and secrete numbers of adhesive and pro-inflammatory molecules that serve to initiate and modulate the primary immune response. Such...
