Akutní selhání ledvin a jeho současná léčbaIntensive Care Medicine

Akutní renální selhání (ARF) se vyskytuje u 5-20 % kriticky nemocných na jednotkách intenzivní péče (JIP/ARO) a signifikantním způsobem ovlivňuje mortalitu a morbiditu, zejména jako součást multiorgánové dysfunkce/selhání (MODS) [1, 2]. ARF je třetí nejčastěji se vyskytující orgánovou dysfunkcí u kriticky nemocných v souvislosti s těžkou sepsí/septickým šokem a v konečném výsledku výrazně zhoršuje prognózu nemocných quo ad vitam, a sice u nemocných, u nichž bylo nutné použít podporu nebo náhradu funkce ledvin [3, 4]. U nemocných s MODS může časné rozpoznání akutní renální dysfunkce/selhání a časně zahájená podpora nebo náhrada funkce ledvin pozitivně ovlivnit potenciální progresi ARF, rozvoj komplikací a ve svém důsledku zvýšit i šanci na přežití [5, 6].
