EDITORIAL World Anaesthesia Day | 137 / Anest intenziv Med. 2026;37(3):137–138 / ANESTEZIOLOGIE A INTENZIVNÍ MEDICÍNA www.aimjournal.cz https://doi.org/10.36290/aim.2026.037 World Anaesthesia Day Asi již od r. 1903 se vždy 16. října slavnostně připomíná Mortonova úspěšná demonstrace éterové anestezie. Nejprve jako Ether Day v Massachusetts General Hospital v americkém Bostonu, kde k ní poprvé došlo. Základní manévr, který nyní používáme pro udržení průchodnosti dýchacích cest, tj. záklon hlavy a předsunutí dolní čelisti, poprvé popsal pravděpodobně J. Heiberg v článku „A new expedient in administering chloroform“ v Medical Times v r. 1874 [4]. Sledování krevního tlaku a pulzu během operace zavedl a následně publikoval v roce 1894 A. E. Codman. Stetoskop začal používat k auskultaci srdce a plic během anestezie Robert Kirk v roce 1896. Především z iniciativy členů anesteziologické společnosti později vznikly nové obory a organizace – Urgentní medicína a medicína katastrof a Společnost pro studium a léčbu bolesti. Změnu zaměření odráží i vývoj názvu naší odborné společnosti. [10]. doc. MUDr. Jiří Málek, CSc. EDITORIAL Volby do výboru ČSARIM – pohled zpět i dopředu | 139 / Anest intenziv Med. 2026;37(3):139–140 / ANESTEZIOLOGIE A INTENZIVNÍ MEDICÍNA www.aimjournal.cz https://doi.org/10.36290/aim.2026.040 Volby do výboru ČSARIM – pohled zpět i dopředu Vážené kolegyně, vážení kolegové, s končícím mandátem výboru ČSARIM je namístě vrátit se k tomu, co jsme na jeho začátku slíbili, co se podařilo a co zůstává úkolem pro další období. Programové prohlášení na období 2023–2026 zahrnovalo široké spektrum témat – od vzdělávání, odbornosti a bezpečnosti péče přes oborová data, úhradové mechanismy a právní a etické otázky až po posilování pozice ČSARIM a její ekonomickou stabilitu [1]. Většinu těchto závazků se v uplynulém období podařilo naplnit. Za nejvýznamnější výsledek činnosti končícího výboru považuji zahájení reformy intenzivní a perioperační péče v České republice. Témata, o nichž jsme dlouho diskutovali převážně uvnitř oboru, se stala součástí agendy systémových změn projednávaných s Ministerstvem zdravotnictví, dalšími odbornými společnostmi a plátci péče. Respekt k její mezioborové povaze a k přínosu ostatních odborností se nevylučuje s jednoznačným prosazováním našeho oboru v organizaci IP v ČR. Hlavním úkolem nového výboru nyní bude tuto příležitost využít a převést připravované změny do praxe. Respekt k oboru vytváříme svou každodenní prací, odborností, spolehlivostí a jednáním s pacienty i kolegy ostatních oborů [1]. Musíme umět své odborné a organizační zájmy srozumitelně formulovat, obhájit, prosadit a pevně je ukotvit v pravidlech organizace a financování zdravotních služeb. Volbami do výboru rozhodujeme, komu tuto práci svěříme. Sebelépe formulovaný program nemá velkou hodnotu, pokud se nenajdou lidé ochotní jej naplňovat a věnovat této práci to nejcennější – svůj čas.

The study primarily examines performance variability in adjusting cervical spine rotation and flexion as part of preventive measures. Design, material and methods: An experimental design assessed the ability of the study group to place an unconscious person into the recovery position. A total of 500 participants aged 15 years and older were recruited from health science high schools and universities. Observations focused on body maneuvers such as shoulder and knee grips, spinal rotation, and head tilt. Gyroscopic sensors and an electronic goniometer measured head and cervical spine angles, while an experienced paramedic evaluated the positioning technique. Results: Correct shoulder and knee grips were used by 445 (89%) and 430 (86%) participants, respectively. Suboptimal grips – e.g., wrist or ankle – were observed in the rest, compromising control and stability. Spinal rotation of the patient was measured and evaluated: 170 (34%) participants showed asymmetry due to poor coordination. Study-specific exploratory ranges based on a subjective assessment of one standardized patient were derived at 20–40° (achieved by 347 participants) for head tilt and 90–110° (achieved by 309 participants) for rotation around the body axis. Conclusion: Considerable variability was observed in recovery-position placement, particularly in head tilt and body rotation. Further research is needed to define the optimal positioning angles and to assess the extent to which trained individuals are able to achieve them consistently in practice. PŮVODNÍ PRÁCE / ORIGINAL PAPER Experimental assessment of recovery position placement by non-medical personnel: a performance-variability study 144 | ANESTEZIOLOGIE A INTENZIVNÍ MEDICÍNA / Anest intenziv Med. 2026;37(3):143-148 / www.aimjournal.cz Výsledky: Správné uchopení ramen a kolen použilo 445 (89 %) a 430 (86 %) účastníků. Background Positioning an injured person is one of the fundamental steps in providing first aid, which all those present at an emergency are obliged to do, both legally and morally. Proper positioning can maintain the casualty’s vital signs until emergency medical services arrive, whereas incorrect techniques may result in significant harm. Some of the basic positions used in practice include the recovery position, the seated position with upper limb support, and the supine position [1]. It is the recovery position that helps to maintain a patent airway in an unconscious person and prevents aspiration of gastric contents. It can be used in both children and adults, but always assuming spontaneous breathing and circulation are present (i.e., it does not meet the conditions for initiating resuscitation) [2, 3]. Currently, there are still few conclusive studies on the optimal recovery position. Its use was recommended by the European Resuscitation Council (ERC) as early as 1992 [1, 4]. The 2021 and 2025 ERC guidelines recommend to: kneel beside the casualty, make sure that both their legs are straight and parallel; place the nearer upper limb at a right angle to the body with the palm of the hand facing up; bring the more distant upper limb across the chest and hold the back of the hand against the face of the casualty; with the other upper limb, grasp the more distant lower limb just above the knee and pull upwards, keeping the leg on the ground; keep the hand pressed against the face and pull on the far lower limb to roll the casualty onto his or her side; adjust the upper part of the lower limb so that the hip and knee are bent at right angles; perform a head tilt and ensure that the airway remains clear; if necessary, adjust the arm under the cheek so that the head remains angled and pointing downwards, i.e., to allow liquid substances to drain from the mouth; periodically check the sufferer to ensure that they are breathing normally [5, 6, 7]. It is advisable to use a blanket or isothermal foil to prevent heat loss. We place those who are vomiting (e.g., an intoxicated person) or bleeding from the mouth or nose into the recovery position. We never leave the affected person unattended, only when treating other affected people, if we have to leave the casualty for a reason, for example to summon professional help or to treat other injured persons [1, 8]. Airway clearance and maintenance is one of the essential actions that must be performed by a lay rescuer prior to the arrival of emergency medical services. This skill should be mastered by every person. Medical dispatchers are able to guide the rescuer step by step on exactly what to do to clear the unconscious person’s airway by advancing the jaw and tilting the head slightly. In the same way, the airway is cleared in the recovery position [1]. Prevention of aspiration, i.e., adequate positioning into the recovery position, is of primary importance in a person with impaired consciousness [5, 6, 7]. This performance-variability study analyzes the variations and degree of consistency in the application of techniques used by the study group to place an injured person into the recovery position. The study primarily examines performance variability in adjusting cervical spine rotation and flexion as part of preventive measures. We started from the hypothesis that the practical performance of the procedure by the study group would exhibit a significant degree of variability (inconsistency). Methods Study Design. This study employed an experimental design to assess the practical skills of trained non-medical personnel in placing an unconscious individual into the recovery position, with a specific focus on evaluating their technique for turning the person over and moving the head within the cervical spine. A total of 500 participants aged 15 years and older were recruited from health science high schools and university programs. Participation was voluntary, with no financial or other incentives. Prior to study commencement, participants were informed that they could withdraw from the experiment at any time without providing a reason. The STROBE guideline for observational studies is used for data presentation. Participants. Each participant completed an initial anonymous questionnaire to gather background information on their education, previous experience in placing individuals into the recovery position, familiarity with airway obstruction risks (such as aspiration in cases of nausea and vomiting), and knowledge of associated complications. The questionnaire primarily used dichotomous (yes/no) questions, with multiple-choice options available for questions regarding the source of their knowledge on the recovery position. Setting. In the experimental phase, each participant was asked to position a standardized patient, representing an unconscious individual,

The standardized patient used in this study was represented by a 20-year-old male with a height of 185 cm and a weight of 87 kg, simulating a fully relaxed, unresponsive state. Throughout the experiment, only one person was used as a standardized patient. Participants were not provided any assistance from the standardized patient, ensuring that all actions taken were solely performed by the participant. All participants who indicated unfamiliarity with the recovery position were provided with a brief theoretical overview, including its purpose and correct technique, and were shown an illustrative reference from the 2021 and 2025 European Resuscitation Council guidelines [5, 7] prior to beginning the practical experiment. Based on subjective assessment prior to testing, study-specific exploratory reference ranges were derived from one standardized patient who reported adequate comfort and spontaneous breathing. If the patient began to feel restricted breathing, these values were rated as inadequate. The study-specific exploratory reference ranges were measured and determined at a head tilt of 20–40° and a rotation around the body axis of 90–110°. Variables. The procedure evaluated specific body maneuvers, such as pelvis and shoulder grips, handling of the standardized patient’s clothing, spinal rotation, and head tilt. Gyroscopic sensors (3D tutor, model 3DT100 by MediTouch Limited) monitored these actions, while the MediTutor system in the MediTouch application (version 10) recorded performance metrics. The sensors were positioned strategically: one on the standardized patient’s forehead (centered between the eyebrows) to track overall body rotation, and another on the back of the head to capture both head rotation and tilt. A certified electronic goniometer (PS-2137) manufactured by PASCO and the PASPORT Angle Sensor PS-2139 with SPARKvue software (version 1.8) provided secondary head tilt measurements by placement on the standardized patient at the level of the seventh cervical vertebra (vertebra prominens). The gyroscopic sensors provided precise measurements of the head and cervical spine angles, capturing both rotation and tilt during positioning. The procedure itself was performed under the supervision of an experienced paramedic. No objective assessment of airway patency, secretion drainage, or ventilation parameters was performed. Statistical Analysis. Descriptive statistics were used to summarize the data. The results are presented as absolute numbers (n) and percentages (%). Language Editing. Results Practical Performance and Observations Participants were tasked with moving a standardized patient from a supine to a recovery position. The critical elements evaluated included gripping technique, spinal alignment during rotation, and head tilt. Grip Techniques Controlled rotation was achieved by 445 participants (89%) using correct shoulder and knee grips. Conversely, 11% used suboptimal grips, such as the wrist, arm, or waist. A correct knee grip from the outside of the knee joint was achieved by 430 participants (86%), while the rest used less effective grip points, such as the thigh, calf, or ankle. Spinal Rotation Spinal rotation was evaluated to assess alignment and segmental twisting during the maneuver. A total of 170 (34%) participants showed asymmetrical rotation associated with the coordination of shoulder and knee pulls. Head Tilt Analysis Gyroscopic sensors and an electronic goniometer identified a study-specific exploratory observed head tilt range of 20°–40° based on subjective comfort assessment: 10°–20° tilt: 130 participants (26%); 20°–30° tilt: 226 participants (45.2%) — study-specific exploratory reference range; 30°–40° tilt: 121 participants (24.2%) — study-specific exploratory reference range; 40°–52° tilt: 23 participants (4.6%). When the angle was set at 20°–40°, the standardized patient reported the greatest subjective comfort during spontaneous breathing. For the standardized patient, head positions at angles less than 20° and greater than 40° were associated with subjective discomfort and perceived breathing restriction. Spinal Rotation and Head Tilt Relationship The study-specific exploratory rotation range derived from one standardized patient was determined to be between 90–110°; within this range, the standardized patient reported subjective comfort. This range was achieved by 309 participants (61.8%). Figure 2 illustrates the relationship between the angles of spinal rotation and head tilt. Fig. 1. Gyroscope placement and motion evaluation
PŮVODNÍ PRÁCE / ORIGINAL PAPER Experimental assessment of recovery position placement by non-medical personnel: a performance-variability study 146 | ANESTEZIOLOGIE A INTENZIVNÍ MEDICÍNA / Anest intenziv Med. 2026;37(3):143-148 / www.aimjournal.cz Behavioral Observations A behavioral trend was noted among younger participants with no first aid experience who demonstrated reticence in physical engagement with the standardized patient, possibly related to discomfort with unfamiliar patient contact. Discussion This performance-variability study established study-specific exploratory reference ranges of 20°–40° and 90°–110° based on subjective assessment of one standardized patient. These ranges were achieved by 69.4% and 61.8% of participants, respectively. The findings are difficult to contextualize because of the limited number of studies evaluating biomechanical aspects of the recovery position. Research has shown that the recovery position does not result in congestion or circulatory arrest in the limbs, supporting its use for maintaining circulation [9, 10, 11]. The 2021 and 2025 ERC Resuscitation guidelines recommend placing adults and children with decreased responsiveness into a lateral recovery position rather than leaving them supine, provided they do not require rescue breathing or CPR [3, 5, 7]. The recommendation further emphasizes that patients in the recovery position should be continuously monitored to ensure airway patency and adequate breathing; if either deteriorates, repositioning to supine and starting CPR may be necessary. A systematic review on this practice has been recommended by the ILCOR First Aid Task Force, particularly for cases where delayed circulatory arrest is possible. Recommendations for further research arise from insufficient evidence at this time to make a clear recommendation [5, 6, 7, 10]. Notably, some studies indicate that starting CPR may be delayed if the individual is in the recovery position [12]. Additionally, emerging studies focus on the challenges of handling unconscious patients in trauma scenarios, particularly during transport or extrication to a supine position [13]. However, there remains a lack of significant focus on the risks of regurgitation, vomiting, or excessive salivation in unconscious patients, which underscores the importance of positioning to prevent aspiration. Our findings are consistent with the view that the recovery position is a crucial intervention in layperson first aid. The results underscore a major gap in the understanding of the recovery position within our sample, with 61.4 % of the participants (n = 307) unfamiliar with the position, and 38.6 % (n = 193) who were familiar, but only 34.8 % (n = 174) Tab. 1. PŮVODNÍ PRÁCE / ORIGINAL PAPER Experimental assessment of recovery position placement by non-medical personnel: a performance-variability study | 147 / Anest intenziv Med. 2026;37(3):143-148 / ANESTEZIOLOGIE A INTENZIVNÍ MEDICÍNA www.aimjournal.cz had practiced it. Notably, only 5% of the participants (n = 25) expressed confidence in their knowledge of how to perform it accurately. These findings suggest a need to incorporate mandatory first aid training (e.g., at the primary school level) to establish foundational knowledge of first aid and early emergency responses [8]. However, first aid education for children requires careful consideration of age-appropriate teaching methods and materials [14]. The study also recorded behavioral observations: some participants, particularly the younger ones without prior first-aid experience, showed hesitation toward physical contact with the standardized patient. Such reluctance may have several underlying causes, including personality, previous experience, and social or contextual factors described in the literature [15, 16, 17]. Reported barriers such as fear of inappropriate touching or of causing injury can reduce bystander assistance [18, 19], although such findings appear region-specific and were not confirmed in the Czech data [20, 21]. This aspect was not systematically assessed in the present study and warrants dedicated investigation. Limitations A limitation of this study is its limited applicability to clinical practice, as it was conducted using a single standardized patient and relied on subjective assessment; thus, it does not account for anatomical and physiological differences among individuals. Additionally, pathophysiological differences may influence the applicability of a single recommended angle across diverse populations. A literature review showed that there are currently no studies defining the most suitable angle of inclination and rotation of the head, and it is not possible to compare the specific results of the present study. Because of a lack of scientific evidence, the determination of optimal ranges is based on a subjective assessment of a standardized patient; it may not be fully appropriate for all affected individuals. Another limitation of the study is the absence of direct assessment of airway patency during movement and objective measurement of secretion drainage. Conclusions This study evaluated head movement during the recovery position in unconscious individuals when performed by the study group. For this study, exploratory study-specific reference ranges of 20–40° for head tilt and 90–110° for spinal rotation were derived from one standardized patient. A total of 347 participants (69.4%) achieved these cervical spine angles when performing the recovery position. These findings suggest that a significant proportion of the study group were able to perform the recovery position and support the need for further research evaluating head positioning during the recovery position.
