NOVÁ DOPORUČENÍ / NEW RECOMMENDATIONS Doporučený postup Evropské společnosti pro anesteziologii a intenzivní péči | 77 / Anest intenziv Med. 2024;35(1):58-80 / ANESTEZIOLOGIE A INTENZIVNÍ MEDICÍNA www.aimjournal.cz Dále nebyly posouzeny výhody ultrazvukem naváděné anesteziologické strategie na výsledky pacientů z hlediska snížení četnosti aspirace žaludečního obsahu a komplikací spojených s bleskovým úvodem do anestezie (rapid sequence induction) u dětí. Je třeba provést další výzkum pro formulaci konkrétních doporučení týkajících se kvality dat získaných s využitím ultrazvuku žaludku a použití ultrazvuku žaludku při klinickém rozhodování. Hlavním cílem předchozích i současných doporučených postupů pro předoperační lačnění bylo snížení rizika aspirace. Bez ohledu na využívaný režim lačnění a skutečný čas lačnění dítěte musíme mít na paměti, že riziko regurgitace nebo aspirace je přítomno při jakékoli sedaci nebo celkové anestezii a je zvýšeno v akutních případech a u kteréhokoliv dítěte s obstrukcí gastrointestinálního traktu. Dalšími hlavními příčinami regurgitace nebo aspirace v době úvodu do anestezie jsou nedostatečná hloubka anestezie nebo problémy v rámci zajištění dýchacích cest; anesteziologové musí být trénováni v rozpoznání a řádném managementu těchto vzácných příhod.
