Awake ECMO však přináší kromě rizik vyplývajících z metody samotné i významná rizika život ohrožujících komplikací – malpozice až úplná dislokace kanyl s masivním krvácením a zástavou průtoku okruhem, riziko plicního poškození způsobeného pacientovou nadměrnou spontánní ventilační aktivitou (patient self‑inflicted lung injury, P‑SILI) u pacientů s ARDS, riziko kardiální dekompenzace při zvýšené zátěži apod. Aktivní rehabilitace je významným rizikovým faktorem uvedených komplikací, proto byla analyzována data 1 157 awake ECLS pacientů z celkem 30 studií o bezpečnosti fyzioterapie v této skupině [28]. Fyzioterapie byla prováděna u všech typů mimotělní podpory i všech typů kanylací, údaje o způsobu fyzioterapie byly dostupné u 676 pacientů, ze kterých 47,9 % (n = 324) podstupovalo pouze rehabilitaci v lůžku, stoje u lůžka bylo schopno 9,3 % (n = 63) pacientů, a 42,7 % (n = 289) pacientů bylo schopno chůze v průběhu mimotělní podpory. Komplikace spojeny s malfunkcí okruhu byly nejčastěji u pacientů na VV ECMO, kanylací femorálního vstupu a v době chůze. Krvácivé komplikace byly spojeny taktéž s VV ECMO, kanylací v horní části těla, v průběhu chůze a u extubovaných pacientů.
